Most likely because 600w was pumped into it without any air moving across it.
Do you really believe there’s a way to be sure? If you play with no airflow, it will smell bad pretty quickly. Your first stinky game will make it look like that.

Okay, I think this is because it was moved around too much, not because it was used normally.
Cheap baskets from China. The ones that look like these are cast, which are generally pretty good, but they’ve been making them thinner and thinner until I guess they found the best one before yours. I think this is an insurance issue, but the company can’t tell what you did or didn’t do to it. If there are two woofers, I would look for cracks in the other one.
Based on the form of the crack, it looks like this was thermally stressed more than once, and then it suddenly moved to the side. That hit had enough bending force to break it in that way.
It’s possible that the speaker has been in the hot sun for a long time and then in full blast AC for a long time. It seems likely that this will happen in a year.
